Dates Requested including drop-off times and pick-up times
Your full name, address (Physical address required and P.O. Box as applicable)
Telephone numbers - Home, Cell and Work
Dog(s) Name(s), age, sex, whether spayed, neutered or intact, color/markings and approximate weight
We will confirm your reservation and e-mail our standard Boarding/Training Agreement and Facility Directions.
We require that reservations be guaranteed with a major credit card (Visa, Master Card, Discover) and must include the following information - credit card number, expiration date and credit card billing address zip code. For your security, DO NOT e-mail credit card information. Call us with that information. If you reach our answering service, leave your full name, telephone number(s) and best time to return your call.
Guaranteed reservations are NON-refundable unless cancelled in writing seven days (7) prior to your dog's scheduled delivery date.
